چکیده Dracocephalum kotschyi Boiss. is an endangered, endemic medicinal plant of Iran with high ethnobotanical and pharmacological importance, particularly in North Khorasan Province. The objective of this review was to compile and analyze existing knowledge on the ethnobotany, botany, ecology, physiology, and medicinal value of D. kotschyi, with emphasis on factors influencing its distribution. The methodology involved a comprehensive review of regional ethnobotanical surveys, ecological assessments, and physiological studies conducted across different elevations and climatic conditions in North Khorasan province. This study was conducted as a narrative review. Outstanding results indicate a very high Relative Frequency of Citation (RFC=0.97) and Use Reports (UR=297), confirming its strong cultural and medicinal significance. Environmental stresses, especially altitude, temperature, drought, and UV radiation, markedly enhanced secondary metabolites such as flavonoids, anthocyanins, and essential oils. These findings highlight the urgent need for conservation strategies and sustainable utilization of this valuable medicinal species.
